As a grower supplying products to a facility working with ProSource Produce, we ask that you diligently work to complete the following modules. Be sure to provide all the necessary information each module requests.  Your efforts are most appreciated.

• Module 1-Complete and Submit Grower/Contact Information Form
• Module 2-Complete and Submit Audit Information Form
• Module 3-Read, Complete, and Sign Non GMO Statement
• Model 4-Select Your Grower ID, Enter Password, and Proceed to Secure Document Upload
• Module 5-Complete Food Safety Program Score and Assessment Form
